PHS students may use the below link to request a visit with a PHS counselor.  Please remember that this form is NOT for emergencies.  If you feel this is an emergency (or even think it could be) please seek assistance immediately in the front office or counseling office.

Contact your teacher, go directly to the front office or the counseling office for assistance.

Requests will be processed within 72 hours after reciept of request. During the summer months requests will be processed within 5 business days.
